  • Approved Driver Application Form
    To be able to drive District vans, drivers have to fill out this form. A current original DMV readout must accompany this form. Along with a copy of a valid California's Drivers License which proves a minimum age of 21. All drivers must complete a safety orientation and demonstrate the ability to safely operate District Vans before being placed on the approved driver's list. Applications must be turned in at least a week in advance. Appointments cannot be scheduled until application is approved. Appointments for vehicle orientation must be made a minimum of 72 hours in advance. If you have any questions regarding this Form, contact the Sarah Neal at x4724.
  • Director’s of Campus Support Report
    Director's of Campus Support should fill out this form in the event an issue arises. The form should be forwarded to Denise Norman at the LAC Facilities Office mail code G9. If you have any questions regarding this Form, contact the Denise Norman at x4684.
  • Facilities Improvements/Office Move Requests
    For Facilities Improvements, you must attach drawings and all relevant information when submitting form. Requestor has to make sure all proper signatures have been completed. Prior to filling out the Form, make sure to read the "How To Fill Out The FIRF Form". If you have any questions regarding this Form, contact the Facilities Service Desk at x4040.
  • Key Issuance/Return
    To request for a key, the requestor must first fill out the Key Issuance/Return Form. Once the form has been filled out with the correct signatures, submit the form to the Facilities Service Desk (LAC - bldg. Z). Your keys will then be ready for pickup after 2-3 days. If you have any questions regarding this Form, contact the Facilities Service Desk at x4040.
  • Move Request Form
    The Facilities Department provides assistance with moves of District equipment and furniture on campus. Moves between campuses require the assistance of the warehouse. All relocation requests require a 72 hour advance notice (do not include weekends in calculating the 72 hours) accompanied by submitting this completed form with the appropriate information needed. Upon completion of this form, forward your request to the Facilities Department (LAC - bldg. Z) for processing. If you have any questions regarding this Form, contact the Operations Manager at x4040.
  • OMNI-CODE Issuance Request Form
    To request an OMNI-CODE, the requestor must first fill out the Omni-CODE Issuance Request Form. Once the form has been filled out with the correct signatures, submit the form to the Facilities Service Desk (LAC - bldg. Z). Your OMNI-CODE will then be ready for pickup after 2-3 days. If you have any questions regarding this Form, contact the Facilities Service Desk at x4040.
  • SET-UP Request Form
    The Facilities Department provides set-ups for District events. However, all set-ups require a 72 hour advanced notice by submitting this completed form. Upon completing this form, forward your request to the Facilities Department (LAC - bldg. Z) for processing. If you have any questions regarding this Form, contact the Operations Manager at x4040.
  • Special Sign Request
    For special sign request, complete this form and clearly provide a diagram or sketch of the sign you are requesting and any additional information needed in regards to location, mounting or delivering of sign. Note that this form requires approval from the Dean, Director or Department Head for processing. If you have any questions regarding this Form, contact the Facilities Service Desk at x4040.
